New Patient Registration

register_now_pink_signPlease contact reception if you wish to register with the practice and one of our staff will be happy to guide you through the process.

When you visit the surgery for the first time you will be asked to complete and sign registration forms, including a questionnaire.  You will also be asked to provide identification for you and anyone you are responsible for.  This information is extremely valuable to assisting us in providing you with the high standard of care we pride ourselves on offering to our patients.

You can be seen at either surgery regardless of where your medical notes are kept. All the doctors attend both premises in rotation throughout the week.  Both surgeries have full access for the disabled
